[WARNING GRAPHIC VIDEO OF A QUAD TENDON RUPTURE!!!] I had to #repost @drnimamehran video. These videos are a good reminder of the TRAUMA that occurs to the structure that is significantly damaged (ruptured). As healthcare professionals we have to remember this is why the patient is apprehensive following surgery, experience PAIN that doesn’t add up to your findings, and why we need to respect the healing process during rehab. These injuries are NO JOKE and require a comprehensive approach during their recovery. •
👨‍🔧 Here is a surgical video demonstrating a quad tendon rupture prior to its repair. In the video, you can see as the knee is flexed there is a large gap from where the tendon tore directly off the superior pole of the patella (the top of the knee cap). Generally, a quad rupture occurs in patients over the age of 40. It usually occurs when the quad is contracting while lengthening under a load (eccentric contraction). In this case, the athlete made a sharp cut on a flexed knee and immediately felt a pop. Tag, Comment, & Share with someone‼️

I’m rolling with @theprehabguys on this one. We can’t prevent injury, but we can train the body to adapt based upon the stresses/forces we place upon it. ・・・
"Encouraging KNEE VALGUS?!" 😱
.
Whether or not you agree with me on this post is okay - you're entitled to your own opinion. But the notion that knee valgus is always bad MUST be put to rest and here's why!
.
Yes, knee valgus is one of the more common positions of an ACL injury and during early rehab for an ACL, it's probably best to avoid a valgus position and teach your patient "ideal" lower extremity alignment. But once they're out of early rehab and in a more strength and conditioning focused program, teaching the patient to CONTROL knee valgus is an absolute MUST. It is a position that they will undoubtedly find themselves in during competition, no matter how well-trained they are in their cutting mechanics. Therefore, it is imperative to teach them to get IN and OUT of a knee valgus position with CONTROL.
.
While there are tons of ways to train this, some of my favorite exercises are curtsey lunges and transverse step up/downs.
.
The curtsey lunge involves dynamically coming in and out of hip internal rotation and adduction. Learning to control the pelvic drop on top of the femur during the curtsey lunge is something that is imperative for advanced athletic movements.
.
The transverse step up should start with pure hip rotation and minimize any transverse loads on the knee. Similar to the curtsey lunge, we are controlling the pelvis on top of the femur. I believe you can load this pattern, but when it comes to actually encouraging valgus in the step down, I believe this should only be done UNLOADED.
.
Teach your patient proper lower extremity alignment and cutting tasks, but also teach them how to move in and out of valgus as it's a position they will 100% find themselves in during competition. Your job as a rehab specialist is to prepare the athlete for ANY demands they might find themselves in on the field. #ACL #valgus #kneevalgus #curtseylunge

🔥Balance Brain teaser🔥 FOLLOW UP.....I loved the ideas from the original post. The common answers were decreased motor control, quad weakness, glute weakness, and foot pronation. •
💡Why after two days does she not shake anymore?! She was able to relearn a movement or recruit more efficiently after reps. Ultimately, she had the the pre-requisite motor control before and was able to relearn it quickly with little practice. Thoughts?! I love the discussion....Tag, Comment, & Share with someone who loves problem solving‼️

💯 FACT....It’s crazy how just observation of my little man has continued to challenge my train of thought. 🤔

🗣 Often times we admire how well a baby/toddler move and always chalk it up to “mobility.” But, if all they had was mobility they would never reach milestones.

👶🏽 They wouldn’t be able to lift their heads, roll, crawl, stand or walk with just mobility. The gains happen when they develop stability of their “degrees of freedom” with strength and coordination.

💡 This is just me thinking out loud but I’m starting to realize that mobility is JUST mobility until you are able to control it at various ranges and positions which require STRENGTH 💪🏽

Just food for thought....What do you think?! Tag, Comment, & Share with someone‼️
