B) Achilles tendon
Testing for this can be done by doing a Thompson test. Having the patient lay prone (on their stomach). Squeeze their calf of the affected side (side of pain or other changes). Normally after squeezing the calf there should be plantar flexion (tippy toe) of the foot. If it's torn -this flexion won't happen. There is a black box with fluoroquinones that there is a potential for tendon rupture. There is increased risk for individuals >60yrs

CORRECT ANSWER:
A) Haldol
Haldol is an older antipsychotics, which are often the most common cause of tardive dyskinesia. Newer antipsychotics are less like to cause TD (but still a risk). TD can also be caused by some anticholinergics, anti seizure, SSRIs, MAOIs, anti-parkinson meds.
Other answers:

Some meds that can cause TD:
Phenytoin (anti-seizure)
Metoclopramide (nausea-stomach)
Thorazine (older antipsychotic)
Levodopa (Anti-parkinsons)
Sertraline (SSRI)
Fluoxetine (SSRI)

CORRECT ANSWER:
B) Hypercapnia (high C02)

High C02 often causes sleepiness, decreased mental alertness, disorientation. Some patients have chronically elevated C02 and their body has become accustomed to these high levels (COPD). Hyponatremia-130 is not very severe. As it gets lower there is a increased risk for seizures. Confusion may develop as well but this is generally at lower levels than 130

Hyperkalemia-With high potassium you worry most about the potential for Arrythmias. Look at the kidneys-a decline in function can often be the culprit
Albumin-low albumin levels are generally from malnutrition. Symptoms include fatigue, crapping, weakness.

CORRECT ANSWER
B) Rhabdomyolysis.
This condition is often seen after falls. Statins (such as simvastatin) are also common culprits and should be held. These patients may often have muscle aches (myalgias), tea colored urine, acute renal failure, elevated liver enzymes, and an elevated CK. Generally with Rhabdo you will see a CK that is 3-5+ times the norm. CK levels generally increase "12 hrs of muscle injury and peak in 24-36 hours" (emedicine ref #1). Aggressive fluid hydration and renal consult are generally a common course of treatment. Rule of thumb: if patient comes in with ARF+ fall look at the CK level. It is often seen after seizures as well. Rhabdo is often missed in the ER and early admission. Rock star status for sure if you catch it when others haven't 😊
