La storia di Taylor Jackson è diventata famosa in tutto il mondo anche perché si tratta di uno dei pochissimi casi documentati (non in modo poi così approfondito), risolti con successo, di "decapitazione interna". Nello specifico, a seguito di uno scontro frontale tra due vetture, Taylor, seduto ed assicurato al suo seggiolino per automobili fissato nel sedile posteriore, subì un urto tale da determinare un'anteriorizzazione della prima vertebra cervicale (atlante) rispetto alla seconda vertebra cervicale (epistrofeo), determinando quindi un allontanamento testa-collo con riduzione del fisiologico diametro del canale spinale e conseguente compressione del midollo spinale.

Un caso particolarmente delicato risolto dal neurochirurgo australiano Geoff Askin. Egli stesso, nel discutere dell'intervento e del decorso post-operatorio di Taylor, ammise, oltre al dire che quello fosse stato il primo intervento del genere, che mai si sarebbe aspettato che Tayor avesse potuto ricominciare a camminare o respirare autonomamente a causa della grave lesione subita.

Nello specifico, il Dott. Askin, utilizzando un graft osseo autologo, asportato da una costa di Taylor, articolò nuovamente atlante ed epistrofeo, permettendo quindi un corretto posizionamento del midollo spinale all'interno dell'omonimo canale.
Taylor fu quindi immobilizzato con una struttura chiamata Halo-Vest che è ciò che vedete nell'immagine. Questo immobilizzatore è pensato per garantire la massima limitazione articolare possibile della porzione più craniale della colonna vertebrale (rachide cervicale), prevenendo qualunque possibile danneggiamento al midollo spinale. Inoltre, rispetto ad altri immobilizzatori, garantisce una maggiore mobilità mandibolare. -Extrasystole🎨

Neuropatia Diabética
A diabetes mellitus é uma doença muito comum na atualidade e com ela pode haver desenvolvimento de disfunções em vários sistemas do organismo, incluindo o sistema nervoso. Entre outros órgãos, os nervos periféricos são geralmente afetados com o desenvolvimento da doença ao longo da vida e por isso manifesta-se mais frequentemente nos idosos.
Os nervos dos membros são responsáveis por levar os impulsos nervosos pela sensibilidade ao tato, pressão, temperatura e vibração, além de controlar os músculos e glândulas do suor, mantendo seu bom funcionamento.
A neuropatia periférica do diabetes ocorre mais frequentemente na extremidade dos nervos mais longos, como nas pernas e braços, mas também pode ocorrer no tórax, face e regiões genitais.
Sintomas
Os sintomas mais frequentes são as dores em queimacao e formigamento em ambos os pés e ocasionalmente também nas mãos.
Sintomas neurovegetativos também podem estar presentes, como hipotensão postural (queda repentina da pressão arterial ao levantar), tonturas, impotência sexual, disfunção de transpiração e até mal funcionamento da mobilidade do estomago (gastroparesia) ocasionando sensação prolongada de repleção abdominal.
Tratamento
O controle rigoroso dos níveis de glicemia (nível de glicose no sangue), com medidas variadas desde dieta até uso de medicação, diminui a progressão da doença, mas em geral não faz regredir os sintomas da neuropatia diabética que trazem sofrimento e piora da qualidade de vida.

Fun exercise/activity I like to use in clinic to break people out of premeditated and guarded spinal movements. The stick is a novel external stimulus the patient has to react to. This often decreases back pain with movement because it changes the context and takes their focus away from their back and the movement itself!

My 64 years old woman patient's scoliosis was causing her walk maximum 10 meters because of discomfort and pain. But after carefully planned and successfuly executed treatment, she can now walk as much as she want with no pain. 💖🤶🏻🛍

SHIN SPLINT RELEASE via. LACROSSE BALL AND 5lb PLATE

Shin splint a.k.a Medial Tibial Stress Syndrome is known as the pain in the inner lower leg. It can be located just behind the shin (tibia) or just above.
-
🔹Some of the causes:
•Change of training surface
•Change of training intensity
•Increase in jumping activities
•Overpronation of the feet

The factors listed above usually cause an overuse pain to the muscles of the medial anterior/posterior lower leg.

🔸Some of the muscles involved:
•Soleus
•Anterior Tibialis
•Posterior Tibialis

1️⃣Use the lacrosse ball on the outer side and the plate on the inner side to sandwich 🍔the tissues behind the shin (tibia).
2️⃣Perform an active release by maintaining the tissue pinned between the plate and ball then moving your foot up and down.

This 👆🏽release will allow for an increase in blood flow and stretch to help decrease the pain and tenderness after activity.

This issue is common among Runners and Basketball players.

Hip bursitis is often a misused term to describe and make medical hip pain without explanation through traditional pathoanatomical approach. Hip bursitis does occur but is fairly rare unless you have direct trauma to the hip. Often this can be do to radiating pain from lower back disorders mechanical in nature. Traditional pathoanatomical diagnosis can be good but it's very much trial and error and can take a long time to have results and physicians often know how to treat through medication, injectable or pill, and surgery. These options don't often lead to much relief. Also labeling this pain as hip bursitis often leads patients to getting poor care because they Google hip bursitis or see someone who treats very traditionally off of scripts vs evaluation. Lateral leg raises, also referred to as hip abduction uses your body weight as the resistance. This single exercise utilizes multiple muscle groups that help to support the pelvis with a focus on gluteus medius when done properly. They are easy to do but proper form is essential to injury prevention. Directions on how to do this exercise can be found below. .
- Lay on your side with your legs and torso in line with one another
- Keep your hips parallel
- Slightly turn your top foot inward.
- Raise leg to 30/45 degrees - Engage the gluteal muscle throughout
Repeat 10x/leg for 3 sets
