Did you know that your shoulder pain could be caused by poor breathing patterns? Here we have a 15 year old elite swimmer who began to have left shoulder pain while doing the freestyle but not while doing other strokes. When looking at his breathing patterns, we noticed a very wide sternal angle at rest, along with a very flared ribcage. This is a very common dysfunction in swimmers due to the nature of them holding their breath and typically having very large lung capacity. We discovered that his issue was his inability to control his ribcage with his left abdominal wall and left serratus anterior which put his left shoulder in a compromised position when coming overhead. This pain came on only with the unilateral stroke of freestyle because it includes a rotational and counter-rotation moment that his other strokes don’t have. A few weeks of adjusting his training volume while teaching him to use his left abdominal wall with his serratus and he was back in the pool pain-free!! His doc ordered 18 visits of PT but he was better in 5, saving this athlete time and money by getting him back in the pool quicker. This is a terrific example of why we must not just look at the area where the pain is in order to find the real cause.
