Another ‘Hands Off’ birth, and what’s that??? The umbilical cord around the neck... 5 TIMES!!!!! And the midwife instructs the Mumma how to untangle babe. THIS is excellent!!! But if it is, there's no shame in that. The medicalisation of birth by the fathers of medicine have done their best to heighten our sense of fear surrounding birth.

For example, how many of us believe that a baby with a cord around its neck is going to choke or be strangled?

This cannot happen because your baby isn't breathing yet. The placenta provides oxygen, not the lungs. "But what if the wrap squeezes the cord or, strangles the cord?" Our bodies are amazingly capable and as such a healthy cord is filled with Wharton's jelly. This slippery gelatinous substance protects the blood vessels from compression and thus inhibits strangulation of the cord.

But...what if it shortens the cord and stops my baby from descending down the birth canal.

To be born, your baby needs only a few extra centimetres. This will depend on placental position and can even be helped by standing, squatting or being on your knees to give birth.
Just keep in mind that your entire uterus is pushing downwards, contracting. It's, not just baby that is heading down. .

Nuchal cord with multiple loops!
This is a case of a post dated pregnancy in a primigravida. She was taken up for immediate c-section due to abnormal cardiotocography, which showed fetal heart rate tracing abnormalities, specifically abrupt decreases in the heart rate below the baseline of varying depth and duration (variable decelerations). There were four loops of cord seen wrung around the baby’s neck.
The baby was cyanotic but survived after the cord has been unwrapped immediately after delivery. She cried after resuscitation and liquor was meconium stained. Both the mother and baby were discharged uneventfully.
A loop of umbilical cord around the fetal neck (nuchal cord) is a common finding at delivery and is dangerous when it is wrapped very tightly and compresses the neck 360 degrees.
It is caused by movement of the fetus through a loop of cord.
In most cases, it is not associated with a significant increase in the rate of any clinically important adverse fetal/neonatal outcome.
In others, tight nuchal cords have been associated with adverse outcomes, including lowered blood flow, hypoxia (low oxygen levels), fetal asphyxia and demise.
Once delivered, the cord can be unwrapped and left intact to allow the trapped blood and the placental transfusion to return to the baby. Resuscitation with ventilation may be needed while this is taking place over the next 3 to 5 minutes.

Double Umbilical Cord Knots - The umbilical cord length averages 19-21 inches and averages 2 to 2 ½ cm in thickness. It contains 2 arteries and one vein. It also contains a lot of a jelly-like substance called Wharton’s Jelly which protects the blood vessels and helps to decrease the possibility of them getting twisted or occluded. *
Umbilical cord knots do happen occasionally when a fetus swims through a loop in the cord, but the structure of the cord makes it difficult for this to have any adverse effects. There has to be a great deal of tension in order for the knot to compress the cord with any significance, and therefore, impede blood flow. *

Double umbilical cord knots occur in slightly less than 1% of all pregnancies! The obstetrician kindly slipped his finger through one of the knots and lifted the cord off the second knot so I could capture this photo. Despite some decelerations of the baby's heartbeat toward the latter part of labor, he was perfectly healthy and in his Papa's arms within minutes after his birth. *

It’s rare that birth photographers are permitted into theatre, and even more rare that we are allowed to document from the ‘business’ end (usually I’m only permitted to stand up by the clients head and wait for the drape to be lowered). I am thrilled to be able to provide my clients with images of their son’s entrance into this world, connecting them to this moment - because #birthmatters !

Many people believe that a baby is being strangled by their cord, however, this is not the case as the baby is receiving oxygen FROM the cord which is still attached to the placenta (#delayedcordclamping). The truth about nucal cords
Did you know, nucal cords(umbilical cord wrapped around the neck) are actually quite common and rarely dangerous? They occur in about one third of births and usually resolve on their own. Don’t lead with fear but knowledge.
