Taking the first quiz of the last term today. Feels good to say that shit. Only about 15-20 exams left before we're done with school and get to take our state test. I was feeling motivated, so I came to the campus early to study 📚📖 . . and make a payment 😒😂💰

“Don’t wait until you’ve reached your goal to be proud of yourself. Be proud every step you take toward reaching that goal.”

These last few weeks people keep asking me “aren’t you so excited to be done, omg you’re a nurse, how excited are you right now?” Honestly, I’m scared and nervous as hell to take my boards for my license, but will ever be ready to take this important of a test that your future is relying on.. ⠀

But also what I’m forgetting is how hard I’ve worked, how much I’ve learned, and how much I have gone through to get to this point. I just keep telling myself to breathe and know that I am ready, I am prepared, and I am confident that I will be an amazing nurse. ⠀

⚫⚫⚫Understanding Hepatitis B⚫⚫⚫ ✔Caused by hepatitis B virus (HBV)
✔transmitted by: ➡️Contaminated blood products or articles (e.g., toothbrush, razor, needles).
➡️Other body secretions (e.g., saliva, semen, urine).
➡️Introduction of infectious material into eye, oral cavity, lacerations, or vagina.
➡️Shared contaminated needles. ✔Serum studies:
📍HBsAg---> indicates INFECTIOUS state
📍HBeAg---> virus is ACTIVELY REPLICATING in the liver of the patient; HIGH RISK of TRANSMITTING THE DISEASE 📍Anti-HBc---> 1st antibody detected. Why is there no HBcAg detected before the appearance of this antibody? Because the antigen is present in the LIVER
📍Anti-HBe---> LOW VIRAL REPLICATION; Patient is NO LONGER INFECTIOUS
📍Anti-HBs---> the patient is now protected from the virus. Determines IMMUNITY due to PAST INFECTION or VACCINATION. ✔Vaccine: three doses of hepatitis B vaccine provide immunity in 90% of health adults; dosing interval----- initial dose, 1 month, 6 months (0---> 1 ---> 6)

👩‍⚕️PERIPHERAL VASCULAR DISEASES👩‍⚕️♦️♦️♦️Thromboangiitis Obliterans (Buerger's Disease is caused by acute and chronic inflammation of medium and small-sized arteries, principally the tibial and radial arteries. Incidence is highest in YOUNG ADULT, MALE WHO SMOKE. Clinical features may include INTERMITTENT CLAUDICATION or leg pain induced by exercise and INSTEP CLAUDICATION or instep foot pain induced by exercise. ♦️♦️♦️Raynaud's Phenomenon results from exagerrated VASOCONSTRICTION of the arteries and arterioles in the extremities, particularly FINGERS and TOES. The restricted flow induces PAROXYSMAL PALLOR and even cyanosis in severe cases. Classically showing "red, white, and blue" changes from most proximal to distal, correlating with proximal vasodilation and central vasoconstriction. Primary Raynaud's Phenomenon is caused by COLD and EMOTIONAL STRESS. It is also more common among young women. ⭕⭕⭕ Thrombophlebitis/ DVT involves the DEEP LEG VEINS. PROLONGED IMMOBILIZATION leading to venous stasis is the most important risk factor. Thrombus may dislodge --> emboli --> travel towards the pulmonary artery --> PULMONARY EMBOLISM ⚠️⚠️⚠️ Other risk factors may include: post-operative state, CHF, pregnancy, and obesity. Homan's sign is pain upon squeezing calf muscles and in dorsiflexion of the foot.
⭕⭕⭕Varicose veins refer to abnormally dilated, tortuous veins. Incompetence of valves leads to stasis, congestion, edema, pain, and thrombosis. Other varicosities are: Esophaheal varices and Hemorrhoids.

Chronic Obstructive Pulmonary Disease (COPD): progressive airflow limitation associated with an inflammatory response.
⭕⭕⭕⭕⭕CHRONIC BRONCHITIS ➡️ MUCUS HYPERSECRETION in large airways associated with HYPERTROPHY OF SUBMUCOSAL GLANDS. Noxious or irritating inhaled substances (e.g., tobacco smoke) causes cellular damage leading to ACUTE and CHRONIC INFLAMMATION which later leads to CHRONIC AIRWAY OBSTRUCTION. ⭕CHRONIC COUGH, dyspnea, cyanosis ("BLUE BLOATERS") 🏵🏵🏵🏵🏵EMPHYSEMA ➡️ DESTROYED ALVEOLAR WALLS cause IMPAIRMENT IN THE DIFFUSION OF GASES ➡️ TRAPS AIR ➡️ DIFFICULT EXHALATION. 🏵Other signs include dyspnea, cough/wheezing, weight loss. 🏵"BARREL CHEST"

⚫⚫⚫ RIPES⚫⚫⚫
💊Major side effects of TB medications. 💊It is essential to MONITOR LIVER ENZYMES because the following drugs can cause HEPATOTOXICITY.
💊Beware of the drug interactions --> Rifampin increases matabolism of corticosteroids, opioids, warfarin, hypoglycemics, and oral contraceptives. ➡️ patient might want to change method of contraception. 💊Isoniazid (INH) inhibits phenytoin metabolism. ⚠️⚠️⚠️ Monitor ADHERENCE to medication. ⚠️⚠️⚠️ #nurse #nurses #nurselife #nurseonduty #nursingschool #nursingstudent #nursingstudentproblems #nursingstudentlife #nursing #nclex #nclexrn #nclexprep #nclexready #nclexpass #futurenurse #futurern #np #futurenp #nursingpharm #nursepharm #pharm #meds #nursepractitioner

💊HMG-CoA Reductase is the rate-limiting step in Cholesterol synthesis
💊HMG-CoA Reductase converts HMG-CoA to Mevalonate which is the precursor of cholesterol
💊HMG-CoA Reductase Inhibitor "-statins" inhibits this step (conversion of HMG-CoA to Mevalonate) so there will be no Cholesterol synthesis ✔Statins are given at night since cholesterol synthesis happens during the wee hours of night
✔Statins are ⚠️HEPATOTOXIC so make sure to MONITOR LIVER ENZYMES prior to and after giving the medication. ✔Statins can also cause ⚠️RHABDOMYOLYSIS so make sure to MONITOR CREATININE KINASE/CK level. Instruct the patient about its symptoms like MUSCLE PAIN, FEVER, and DARK URINE. It must be reported promptly to the care provider.

💙Heart Failure/ Congestive Heart Failure (CHF) occurs when the heart is unable to pump blood at a rate sufficient to meet the metabolic demands of the tissue. 💙It is a common end-stage of many forms of chronic heart disease; often develop from chronic work overload or ischemic heart disease. 💙LEFT-SIDED CHF can lead to PASSIVE CONGESTION (blood backing up in the pulmonary ciculation), STASIS OF BLOOD in the left-side chambers, and INADEQUATE PERFUSION of downstream tissues leading to organ dysfunction. 💙Hallmark sign of RIGHT-SIDED CHF is PEDAL/ PRETIBIAL EDEMA.

💊Angiotensin Converting Enzymes Inhibitor (ACEI) inhibits the conversion of Angiotensin I to Angiotensin II, blocking vasoconstriction and fluid retention from aldosterone secretion. 💊This drug is commonly used for hypertensive patients.
💊One of the most common adverse effect of this medication is DRY COUGH
💊A small percentage may develop life-threatening Angioedema.

1. Check the APICAL PULSE prior to administration: WITHHOLD dose and contact care provider if rate is <60 bpm.
2. Encourage intake of potassium-rich foods.
3. Assess for signs of impending toxicity (xanthopsia/ yellow vision, HYPOkalemia) --> treated with Digibind.
4. Monitor for hypokalemia (<3.5meq/L)
5. Instruct to count radial pulse and record before each administration.
6. Therapeutic level: 0.5- 2.0 ng/ml

As nurses, we should always remember the normal values of the major electrolytes. ⚫Sodium (Na+) -most abundant cation in ECF
-action potential, basic to communication bet. nerves and muscles
-regulates acid-base balance, sodium and hydrogen exchange in kidneys
-sodium-rich foods: processed foods, snack foods, condiments, smoked meats, and aged cheese ⚫Potassium (K+) -most abundant cation in ICF -resting polarization and repolarization of nerve and muscle fibers
-potassium imbalances may lead to muscle paralysis and cardiac irregularities ⚠️
-potassium-rich foods: bananas, avocados, orange, dates, apricots, cantaloupe, potatoes, and raisins ⚫Calcium (Ca2+) -strengthens the bone and teeth
-required for functioning of nerves and muscles
-HYPERcalcemia: nervous system becomes depressed and sluggish --> decreased DTR; may also lead to the formation of renal calculi
-HYPOcalcemia: nervouse system becomes excitable --> cramps, tetany, hyperactive DTR, positive Chvostek's and Trousseau's signs
-required for blood clotting
-calcium-rich foods: milk, dairy products, whole grains, legumes, leafy green vegetables ⚫Magnesium (Mg2+) -cofactor for many enzymes involved in energy metabolism
-constituent of bone
-magnesium-rich foods: nuts, soybeans, cocoa, seafood, while grains, dried beans, and peas ⚫Chloride( Cl-) -most abundant anion in ECF
-major component of gastric secretions
-dietary source--> salt ⚫Bicarbonate (HCO3-)
-reacts with strong acid to form carbonic acid and basic salt; limits acidosis ⚫Phosphate (PO4 2-) -cellular energy metabolism: phosphate+ ADP+ ATP (energy currency of cell) -combines with calcium--> harder bones
-component of DNA and RNA
