Fun little mystery mold from the other day...We had one colony of a white, cottony mold growing from a BAL (bronchial alveolar lavage). Every slide we made showed these nice round conidia of various size, but we couldn't see where they were coming from 🤔 Even after setting up a slide culture, STILL no good structures 🙄 Our lab identifies molds based on macroscopic and microscopic morphologies, and we were unable to identify it. Our next option would be to send it out to the Fungus Testing Lab in Texas. In the end, we decided not to pursue a full identification because the patient had passed away (from something completely unrelated to the mold isolate). When working up molds from clinical specimens, its important to look at the whole picture. One colony growing on one plate from one culture. Is that significant? Is that contamination? In these situations clinical correlation is needed. Is it a common pathogenic mold, is it growing in multiple cultures, is the patient symptomatic...all key questions that need to be answered. In our case we had ONE colony on ONE plate, and it did not seem to correlate with the patient's condition (that ultimately took his life). It would be of no benefit to send this out to a reference lab for further workup...so unless you guys know what it is, it remains a mystery to me!
