#repost from our pal and fellow PA @mrs_angemi showing an amazing enbloc resection of bilateral kidneys, ureters, and bladder, all taken for polycystic kidney disease. Kidneys with this genetic disorder often get the size of a football--or bigger--due to the multiple cysts that continue to grow and fill with fluid, eventually replacing the entire kidney parenchyma. Patients often need dialysis to help filter their blood, if--or until--a transplant is available. This is just one of the many disorders of the kidney that we see as PA's that isn't a cancerous tumor. #kidney #polycystickidneydisease #lifeofaPA

Today's lesson: mandibulectomy specimens and cancer of the oral cavity. Chewing tobacco can lead to some very unpleasant side effects, kids. This photograph shows a partial mandibulectomy specimen for Squamous Cell Carcinoma. And yes, for the untrained eye, you're seeing an indentation (i.e.: the cancerous lesion) at the level of the right jawline that involves the skin, complete with a removal of part of the underlying jawbone (mandible) with some intact teeth. Although SCC's caused by tobacco use are not the only reason we see mandible specimens in Pathology, it's a proven fact that patients with a long history of chewing tobacco have been shown to be at a higher risk for developing SCC of the floor of the mouth, tongue, and oral cavity. Cases like this one are assessed by the PA and Pathologist for margins along the skin, soft tissue, mucosal surfaces (along the gumline), and at both the ends of the bone, as well as looking at microscopic sections of the tumor itself. Patients often also have multiple levels of lymph nodes removed from their neck, which are also assessed for staging to help determine the patient's subsequent treatment plan. ...Before it becomes the most asked question: "yes", this patient IS alive; More than likely, this surgery was not the last one they will have, as reconstruction after such an extensive removal can often take multiple surgeries. #oralcancer #pathology #lifeofaPA #pathassist #mandible #scc #dontchewtobacco #cancer
