Patient was diagnosed with a Morton’s neuroma. Why were films ordered? 🤷🏻‍♀️
Do you see it?!!! 5th toe looks a little thick if you will. Observe 5 toes, but underneath that skin of the 5th toe...extra phalanges!!! So cool
But guess what’s even cooler...this woman is in her 50’s. Guess who’s adapted to having 6 toes. Yep. Her.
Doc told her it was her extra toe causing her neuroma....removal time 🤔🤔🤔. How about a big NO thanks. No thanks to someone cutting into her foot

This woman works for the airlines. Forced to wear a heel for years. Yes I bitched big time about that. I don’t care if you had 4 toes or 10 toes, shoving your foot into a heel for years can potentially cause a pissed off foot. And in this case a neuroma

We addressed foot function, changed her footwear, got her a neuroma pad to assist that transverse met arch and potentially saved this woman from foot surgery. 🤗
Plus she gets to keep her extra phalanges! God blessed her as far as I’m concerned 😉

3. Pattern assisted first ray work
Multiple uses for that single axis rocker board. So cool. He’s having a difficult time but that’s what’s awesome about our brains. That concept of neuroplasticity. Feed it the right stuff, groove different pathways/patterns and we change the brain = change function. Pain is an experience created by the brain. Change the experience. Make it fun again 👍🏼
1. Big toe on that board assisting it into extension. Head of the first metatarsal stays on the ground
2. Once end range, push that big toe into the board for a good 15 seconds .
3. Then, try to lift it away from the board, keeping little toes on the ground for 15 seconds.
4. Repeat several times

FLIP FLOP....now I’m not saying NEVER wear flip flops..well maybe but it’s because I see all patients with foot pain and instability so here’s the deal

Try walking in a flip flop without toe gripping. It’s hard right? Most flip flops would slip right off your foot. So we ‘toe grip’ or activate the long toe flexors to keep the flop on. Here’s the problem and there are many 🤭. Don’t be mad at me, I’m just stating fact btw.
1. A lot of us, especially those with foot pain have NO problem activating the long toe flexors. That’s how we are stabilizing our bodies. Toe grip, walk. Toe grip, walk. Ie hammer toes. Not ideal.
2. Reciprocal inhibition. If I’m constantly firing my long toe flexors guess what I can’t be firing?? Yep. My long toe extensors. Problem? 💯. Let’s name them:
1. Hammer toes
2. Shin splints (cant strengthen that anterior compartment that’s where my extensors live) leading us to..a tight posterior compartment. All I’m doing is activating my flexors. 😱😭
3. Plantar fasciitis
4. Achilles tendinitis
And don’t forget to go up that chain, what does that big gluteal muscle do, yep ‘extends’ the hip. Not if your fighting to keep that shoe on though
5. Low back pain 🤷🏻‍♀️🤷🏻‍♀️
I know plenty of us walk around in these things and some people have no problems, BUT, if you have the above mentioned issues, I’d think again.
Sandals with a back strap, @vivobarefoot @earthrunners @xeroshoes for example, tend to be better. Less flop 😉. Literally. Nobody wants a floppy anything anyway 💪🏼👊🏼

My favorite 'integration' exercise pre-run. This bad boy gets posterior tibialis by 'tenting' the foot/supination with eccentric control back to midline, VMO and gluteus medius. The slight external rotation while in a hip hinge will get those guys quivering. You will burn out in seconds. My kind of exercise. No more clam shells 😉. No more terminal knee extension for VMO. BORING. Helical rocktape this leg if there is medial collapse with gait and this is a win-win. And if we need help, we get it. So as we are strengthening her foot/hip and restoring her mechanics, we are going to take some load off of her painful tissues. Why? She’s in pain. 😳
Enter ‘orthotic therapy’. That doesn’t mean orthotic attached to your foot til your 6 feet under. That means ‘therapy’. We use it as a tool to help assist the body’s own healing mechanism, similar to taping. Once we restore function, we slowly start to decrease the therapy. Makes sense to me👍🏼🤷🏻‍♀️ Orthotic therapy for this forefoot varus: An orthotic shell ends at the distal met shafts. (Arch) It is only able to help ‘control motion’ ‘provide proprioceptive feedback’ during contact / midstance phase of gait. Now let’s think about this

If our patient (large FF varus) has a problem with toe off /propulsive period of gait, is that shell alone going to help 🙅🏻‍♀️ Negative.
Enter the forefoot varus post. Made by @reidnellesdc 👣👍🏼. We bring the ground up to the big toe decreasing the forefoot slam to the ground....also, decreasing torque through the rearfoot!!!👏🏼👏🏼
Post needs to be flexible which is why we use cork as to not limit dorsiflexion of the MTP joints. Super cool. Seriously. We continue to strengthen the system and slowly decrease our extrinsic modifications. ✅✅
This can be used by itself which I like to try first....less is more. I always go with most minimal correction to achieve result

1. Tibialis posterior. My fave. So many cool insertions. Lifts the arch and inverts rearfoot. Done.
2. Flexor Hallucis Brevis. Impt one here. Create mechanical leverage for big toe. Without this guy working, welcome turf toe into your life. 😭
3. Abductor hallucis, no more bunions!
They all work together to help our patient control pronation. Good stuff.
Now......what about this guy though. Extensor Hallucis Brevis. This one ❤️❤️
Very important for our patient. This one makes our medial tripod complete, look how beautiful that is! Drops the head of the first met BINGO. So exciting
Step 1: bend big toe and keep it bent.
Step 2: keep it there and move it

Okay. Here we go. Patient had a large forefoot varus deformity. Ie. > than 4 degrees. You look at her foot. Her foot looks flat. You throw an orthotic under her foot. Her symptoms get worse. In fact, she ends up getting another tibial stress fracture. What the? Yes. True story

Her foot isn’t flat. It’s weak. Her Intrinsics are toast. Many ways to identify but that print helps too. The problem isn’t in her rearfoot or midfoot. It’s in her forefoot. 😱. By having to get that big toe down and aggressively I might add, she’s bringing the whole foot with her with a lot of force ie. the appearance of a pronated foot. 👣👣
Now look at that video. Watch it but also listen. You can hear it all going down 🙈🙉. Those poor tibias
Now what? The orthotic is going to make it MORE difficult, especially with nothing helping that forefoot. Not to mention, shoving a piece of acrylic into an arch that is forcefully being brought to the ground. OMG. So my thoughts?
She’s a bad ass remember. She’s going to run. I need to decrease load. Change it up. Strengthen the foot, the hip. But I think we need help. @reidnellesdc have been working on some things in the office. What if we helped just the forefoot? Forefoot Varus posts? Flat foot day 3. My top 4 intrinsic strength exercises. To be done....all day. Every day. For the rest of your life. 😉🔹🔹🔹. Every chance you get take those shoes off and do these. At your desk. Sitting. Standing. In your shower. Brushing your teeth. I taught my 6 year old daughter how to do these. She loves seeing how high she can lift that big toe. 👍🏼👏🏼🙌🏽. Couple weeks, already saw change, really cool. Their little developing brains crave this stuff. 🔹🔹. 1. Big toe lifts. Maintaining ‘tripod’. Heel, 5th met and base of first on the ground. Nice beautiful maintained arch. 2. 4 toe lifts. Same deal. 3. Lift all toes, SPREAD your toes as far as you can and REACH them forward and bring em back. 4. ‘Tent’ the foot. Do. Not. Scrunch. Toes. Maintain good control and imagine someone was pulling a string towards the ceiling right at your midfoot. All day. Every day. Not kidding.
