One of the main things I have "changed" lately that have noticeably improved both my strength and size is writing down my workouts for the week FIRST and also focusing on using the muscles I'm trying to work (aka controlling all of my reps). When I write my workouts down; even if I'm tired or feel like being done, I have to keep pushing to finish the workout.
The whole concept behind working out is to push your body past its limits. So whatever makes that happen for you, do that.
