Getting some practice in ahead of the @nomancampaign row from Barcelona to Ibiza this summer. The timing is better than some of my past crews! Same pt as previous post. 45yo female with pruritic vesicles and erosions lower back, neck, upper chest, and inframammary areas. +scalp scale. No mucosal lesions. Diagnosis is pemphigus foliaceus (PF), which is an autoimmune skin disorder characterized by acantholysis of the epidermis (dissolution of the bridges between epidermal cells) resulting in blister formation. The autoantibody is IgG directed against a cell adhesion molecule, desmoglein 1. It is generally a more benign form of pemphigus compared to pemphigus vulgaris, for example. Onset is usually in middle age. May be seen in any age group but tends to be a disease of older adults. Recent sun exposure can exacerbate the condition. Oral lesions are rare, and most patients do not become severely ill. Often chronic and can last for years. On exam, you’ll see superficial flaccid blisters, crusted erosions, and scale on the scalp, face, and trunk, often in a seborrheic distribution. Lesions usually start on the trunk. Lesions start as small, flaccid bullae that rupture almost as they appear, leading to the crusting. Adherent scale crusts may resemble corn flakes. Below each crust is a moist surface with a tendency to bleed. The classic histologic finding is acantholysis in the upper epidermis, usually in the granular layer. Our patients’ biopsy shows a cleavage plane between suprabasilar and subcorneal. It’s difficult to favor folicaceus vs vulgaris based on the level of the split alone, but based on the clinical distribution and relative sparing of the hair follicle on pathology, we favored PF but would need immunofluorescence or ELISA to be totally certain (not available in this patient’s country). If PF is limited in distribution and not severe, potent topical corticosteroids can be tried. If severe, systemic therapy is indicated, usually starting with prednisone. A steroid-sparing agent may also be required.
